Major Advisor Selection Form (Grad) - Use this form for selecting your Major Advisor (this should be accomplished by the end of your second semester).  Please make sure you give the form to Gail after it's completed and signed by you and your Major Advisor. 
 
Major Advisor Change Form (Grad) - Use this form only if you are changing your Major Advisor.  It must be completed, signed, and turned in to Gail promptly.

Teaching/Professional Development Opportunities for Ph.D. Students in Arts & Sciences

Graduate Guide (GSAS) - From the Graduate School. 

Notice of Title, Scope, and Procedure of Dissertation (scroll down to the Dissertation/Degree Forms section to find the link).  PhD students are expected to submit this form at least six (6) months before the month in which the degree is expected to be conferred (August, December, or May) or before the beginning of the 5th year of full-time enrollment in the Graduate School, whichever is earlier.  After completing both sections of this form, give it to Gail in the Main Office. 

Intent to Graduate  (scroll down to the General Forms section to find the link)
You should first try to file your Intent to Graduate form on WebSTAC, then if you cannot do so, file the paper form (using the link above).  Please be sure you give Gail a copy of your completed form whether you file on paper or electronically.  Candidates for a degree must file an Intent to Graduate form for the semester in which they intend to graduate. The Office of Student Records provides this form.  They can also inform you about the deadline for filing.  Please note that, if you do not finish by the time you had originally stated in your first Intent to Graduate form, you must file a subsequent form for when you now intend to complete your work.  You should keep your own copy of this form, as it has diploma pick-up and mailing dates.  This form also includes your mailing address, which may change between the time you fill the form out and when you finish your work; therefore, you should contact the Office of Student Records any time you change addresses after filing this form. 

Verification of Enrollment and Degrees
Students who need verification of their enrollment status or degrees earned at Washington University may obtain and print verification forms at any time from WebSTAC using the Student Self-Service site at the National Student Clearinghouse, at no charge. (Former students who need assistance with the WebSTAC logon may contact the Office of Student Records at 314-935-5959 or student_records@aismail.wustl.edu.)

Important! 
If your advisor wishes to receive a copy of your bound thesis, you need to submit four (4) copies of your finished thesis (not 3, as stated in the guide).  Attach a note to the 4th copy stating that it is for your advisor and that it should be sent to you home department (E&PS) after binding (and it's probably a good idea to add your signature to the note).

Survey of Earned Doctorates

U.S. Copyright Office   The Copyright Office provides expert assistance to Congress on intellectual property matters; advises Congress on anticipated changes in U.S. copyright law; analyzes and assists in drafting copyright legislation and legislative reports and provides and undertakes studies for Congress; and offers advice to Congress on compliance with multilateral agreements, such as the Berne Convention for the Protection of Literary and Artistic Works. The Office works with the Executive Branch’s Department of State, the U.S. Trade Representative’s Office, and the Department of Commerce in providing technical expertise in negotiations for international intellectual property agreements; provides technical assistance to other countries in developing their own copyright laws; and, through its International Copyright Institute, promotes worldwide understanding and cooperation in providing protection for intellectual property.

The Copyright Office is also an office of record, a place where claims to copyright are registered and where documents relating to copyright may be recorded when the requirements of the copyright law are met. The Copyright Office furnishes information about the provisions of the copyright law and the procedures for making registration, explains the operations and practices of the Copyright Office, and reports on facts found in the public records of the Office. The Office also administers the mandatory deposit provisions of the copyright law and the various compulsory licensing provisions of the law, which include collecting royalties.

Additionally, the Copyright Office and the Library of Congress administer the Copyright Arbitration Royalty Panels, which meet for limited times for the purpose of adjusting rates and distributing royalties.

Dissertation Fellowship Certification Form (Note: To be updated when new form is made available)

Applications for 2008-2009 Dissertation Fellowships will not be considered without inclusion of a form submitted by the home department certifying that the applicant has either already completed all Ph.D. requirements other than the dissertation or is expected to complete the requirements by a specified date.  DF awards are for an anticipated terminal year of writing; therefore virtually all of the necessary research should have been completed prior to the start of the award period.  [Note: Olin and Chancellor’s Fellows are not eligible for the Dissertation Fellowship.]
